Consent / Permission Forms
In the course of a typical school year, our students
will have opportunities to participate in field
trips and other off-campus activities.
In order to properly convey this information to the
parents and protect your child while on these
outings, we require that you complete a Parental
Consent / Permission Form giving your child
permission to participate in this activity and
authorizing the school to act in your child's best
interests while they are participating in this
activity.
Below is a link to a blank, generic Parental Consent
/ Permission Form that can be printed and completed
as needed.
